Dental Membership Plan

Adult Plan: $360 (Less than $1 a day!) For patients with healthy mouths without periodontal disease.

Periodontal Plan: $725 For patients with periodontal disease. Plan includes maintenance therapy.

Membership Program Guidelines

  • The patient will need to schedule a two hour new patient exam for $75 to determine if they are a candidate for the adult plan or the periodontal plan.
  • The credit card will be kept on file, and the $75 will be needed to schedule an appointment and is non- refundable.
  • An exam will be conducted by the dentist to determine health and status of mouth and which plan the patient is eligible for.
  • Patients with periodontal disease are not able to sign up for the regular adult plan.
  • If the patient continues with membership then the $75 will be applied to their respective plan.
  • Membership payment needs to be paid in full at the time of sign up and will last for one year from initiation date.
  • Patients 13 years and older are eligible.
  • This plan cannot be used for injuries covered under workers’ compensation claims.
  • HSA/FSA/MSA accounts may be used to pay for membership and treatment.
  • Any diagnosed treatment needs to be completed in year of membership.
  • There are no refunds if a patient decides not to use benefits.
  • This plan cannot be used with any insurance plan or other dental plan.
  • Membership plan does not include orthodontic or botox treatment.
  • This plan only applies to services rendered at High Point Dentistry.
  • The plan is available for renewal after the expiration date, with automatic renewal an option at the time of sign up.
  • No membership card will be given. The plan’s effective date will be kept on file.
  • Patients will receive a signed copy of membership with the expiration date and their selected option of automatic renewal.
  • Participating patients will receive 20% off all dental services including crowns, fillings, and bridges.

Terms and Conditions
Renewal Conditions: By joining, patient is authorizing High Point Dentistry to bill their credit card for the full amount of the appropriate plan. If automatic renewal was not selected at the time of the sign up, patients must notify High Point Dentistry of the continued renewal. Patients that chose automatic renewal will be charged with the card on file the yearly plan amount following the expiration of their current plan. 

Termination Conditions: High Point Dentistry reserves the right to terminate plan members from its plan for any reason, including non-payment, appointment cancellation, or no show. If High Point Dentistry terminates the program or membership for a reason other than non-payment, patient will receive a pro-rata refund of the membership fees.

Missed Appointments and Cancellations: In order to be respectful of the needs of other patients, please be courteous and promptly call High Point Dentistry if unable to show up for a scheduled appointment. If it is necessary to cancel an appointment, we ask that the patient call our office at least 24 hours in advance of the scheduled appointment time. A cancellation will be considered late if a patient fails to cancel their scheduled appointment with a 24-hour advance notice. At High Point Dentistry, we understand emergency situations occur, and we are understanding and cooperative of unforeseen circumstances. Late cancellations will be documented in the dental record. A failure to be present at the time of scheduled appointment will be recorded in the dental record as a “NO SHOW.” Late cancellations and “NO SHOWS” will be charged a $50 fee to the card on file.The plan is subject to termination if a patients has three (3) late cancellations or “NO SHOWS.”

Limitations, Exclusions, and Exceptions: This dental membership plan is offered by High Point Dentistry. The dental membership plan is not a licensed insurer, health maintenance organization, or other underwriter of health care services. No portion of any provider’s fees will be reimbursed or otherwise paid by the dental membership plan. The dental membership plan is not licensed to provide and does not provide medical services or items to individuals. Patients are obligated to pay for the plan and dental services at the time of sign up and agreement. The dental membership plan discounts may not be used in conjunction with any other discount plan or program. All listed or quoted prices are current prices and are subject to change without notice. Eligibility for the dental membership plan include those 13 years old and older.

Payment Terms: Patient will provide High Point Dentistry with valid and updated credit card information. Throughout the duration of the membership, a credit card will remain on file. This authorizes High Point Dentistry to charge the credit card for any services not included in the dental membership plan, late cancellations, “NO SHOWS”, or auto-renewal of the membership.
